Registration Process

Follow these steps:

1. Download and print the Health Certificate and Guest Profile forms. Use the Additional Guest Profile for each additional guest: (Click on each to download)

2. Mail the completed forms along with $25.00 to:

Important Notes:

• Please make checks payable to Paws At Play
• Registration fee is $25.00
• On the Guest Profile, there are two separate forms.  The 2nd form starts with Additional Guest and only has to be filled out if you are registering more than one pet.  Additional pets are $20.00 each.
• A Certificate of health must be filled out by all dogs coming into the facility whether it is for grooming, obedience, boarding, daycare or any other activities. Alternatively, one of our staff members will call your veterinarian for vacine information.
• It is recommended that all guests be registered and set up in our system. (This will speed up the check-in process).
